Sacramental Preparation is an exciting time for your family! There is also a lot of paperwork that we need to collect before your child is able to celebrate the Sacraments of First Reconciliation, Confirmation, and First Eucharist. Your family will be provided with a hard copy of each of the forms that need to be filled out and returned to us, but your or your Sponsor may also download needed forms here.

Completed paperwork may be returned to Shannon Chisholm via email, mailed to the PO box at the top of each form, dropped off at the parish office, or returned with your child during their Sacramental Preparation classes.
